- Do you currently have any heart issues?
- Do you have a family history of heart issues?
- Do you currently have diabetes?
- Do you have a family history of diabetes?
- Do you experience erectile dysfunction (men)?
- Do you smoke or have a history of smoking?
- Have you ever live with someone who smokes?
- Are you overweight?
- Do you eat fried or fatty foods three times a week or more?
- Do you have high cholesterol?
- Do you have a family history of high cholesterol?
- Do you have high blood pressure?
- Do you have a family history of high blood pressure?
- Are you over the age of 40?
- Do you tire or fatigue easily?
- Do you have swelling in your legs or feet?
- Do you have aching, cramping, or pain in the legs while you walk or exercise that goes away after rest?
- Do you have tingling, numbness, or coldness in your hands or feet?
- Do you have loss of hair on your feet or toes?
- Do you have irregular growth of fingernails or toenails?
